Don’t worry about that buddy, just speaking to your local recruiting ofice.

Ask your doctor for copies, or tell your recruiter where your records are, and if you sign a release, they may be able to request them directly.

Talk to the doc that treated you and talk to your recruiter. I just retired a few months ago and entry requirements are probably alot different than when I joined. I joined under the go to jail or join the army plan. Talk to your recruiter to verify what the actually need and then go to your doc. Make sure you get the stuff you actually need. Do not provide anymore or any less than required by the recruiter.

Go to the hospital you were treated at and request them. They might charge for copies though.

Just contact the doctor’s office where you were being tested/treated and they will give them to you. If you can’t physically go there (like if you have moved or something) then you should be able to call them and they’ll probably have you fax a release to them then they’ll mail them to you. That is what I had to do, and it was no hassle at all.
