Talk to the doc that treated you and talk to your recruiter. I just retired a few months ago and entry requirements are probably alot different than when I joined. I joined under the go to jail or join the army plan. Talk to your recruiter to verify what the actually need and then go to your doc. Make sure you get the stuff you actually need. Do not provide anymore or any less than required by the recruiter.</description>

Go to the hospital you were treated at and request them. They might charge for copies though.</description>
