How do I obtain shot records?

I’m applying for college again this fall and they’ve requested a copy of my shot records. I used to be in the military and I’m also a military brat so I know I’ve had ever innoculation known to the western world, but I’m almost 30yrs old and haven’t even seen my shot records since high school. How do I obtain a copy of these?

call your veterinarian’s office

call your local health department facilty for instance I live in tx so I would call tx dept of health they should have it or your local military branch

High school records!
They are usually kept at a different office than the school they would be able to tell you that. If you turned them in than they should have a photocopy in the file.
