1886 - December 31, 1972 (Enlisted) 1903 - December 31, 1972 (Officer) 

Full name, Social Security Number, and/ or Service Number (both when available), date of birth and place of birth (city and state), date of released/discharge from Naval service, address where record is to be mailed, signature of member/Next of Kin, and date.
